The all-okama “second island of women” — the queendom on Momoiro Island (Peachy Island), shown in Ch. 523 (pp. 18-19) as Sanji’s Kuma-scatter landing spot, where its okama give chase (”…I’m in hell”). A deliberate structural mirror to Amazon Lily (the first island of women) — the all-female Kuja rhymed against the all-okama Kamabakka.
Its ruler is supplied in Ch. 532 (p. 14): Bon Clay names “Iva” (Emporio Ivankov) as “the Queen of the Kamabakka Queendom,” a “Miracle Worker” imprisoned in Impel Down — answering Ch. 523’s open question of who rules it.

Kamabakka Queendom (カマバッカ王国 Kamabakka Ōkoku) — kama (from お釜 okama, slang for an effeminate/cross-dressing man) + bakka (ばっか, colloquial ばかり “nothing but / full of”) = “nothing but okama / full of okama,” exactly describing the all-okama nation. 王国 (ōkoku) literally = “kingdom”; the VIZ translation renders it “Queendom” to match the okama “hearts of maidens” framing. It sits on 桃色 (momoiro, “pink”) Momoiro Island; the all-okama “second island of women” deliberately mirrors the all-female Kuja of Amazon Lily.
