Chapter 538: Level 5.5: New Kama Land

Summary

In the hidden level between Levels 5 and 6, the “Miracle Worker” Emporio Ivankov gambles to save the poisoned Luffy: his Emporio Healing Hormone force-stimulates Luffy’s immune system to fight off Magellan’s venom, but at the price of years of life and an agony Luffy can only survive on willpower — “if you live, you win; if you die, the poison wins.” As Luffy screams for hours in a sealed coffin-cell, Ivankov lectures the weeping okama residents that miracles only happen to those who never give up, and reveals his power: the Horm-Horm Fruit, which lets him manipulate hormones at will and remake the human body from the inside (the source of New Kama Land’s gender-transcending “candy” residents). The Newkama explain their hidden paradise — Level 5.5 / New Kama Land — dug out long ago by a prisoner with a digging Devil Fruit, supplied through secret routes that riddle the prison, and informed by stolen newspapers and transponder snails. Bon Clay learns the deeper truth: Impel Down has a sixth, secret floor — Level 6, “the Infinite Hell” — where the government buries prisoners it has erased from history, including his old boss Crocodile, the Warlord Jimbei, and legendary pirates Catalina Devon, San Juan Wolf, and Vasco Shot; even the prison’s former head jailer Shiryu of the Rain was caged there for slaughtering inmates. Worse, Ace — Luffy’s brother — is on Level 6 and will be shipped out in the morning to be executed at 3 p.m. With Luffy needing days to recover, the clock makes rescue impossible — yet after roughly twenty hours of screaming, Luffy detonates his chains and erupts back to life howling for FOOD, cured in under a single day.

• Level 5.5 — New Kama Land (pp. 8–10): a hidden okama “paradise” lodged “somewhere between Level 5 and Level 6.” Dug “a long time ago” by a prisoner whose Devil Fruit “was good for digging”; its residents are originally-lost prisoners. Impel Down “is like a giant ant colony inside a really big rock” — entrances “all over the place” (in the fire, the spikes, the beast-dens, the morgue) feed supply routes the guards never notice (pp. 9–10).
• Emporio Ivankov ate the Horm-Horm Fruit (p. 8): “I am an engineer of the human body who can alter people from the inside… Now I can freely manipulate hormones at will.” Controls “gender, body temperature, pigmentation, growth, excitement.” The fruit is the in-world source of the Newkama’s gender transcendence (“the women become men… men become women”).
• Emporio Healing Hormone (pp. 2–7): Ivankov stimulates Luffy’s “natural immunity factors,” forcing his body to “break down and regenerate at an incredible rate” to fight off the poison — survivable only by enduring the pain. Quoted cost: “ten years of your life,” only a “2 or 3 percent” guarantee; “but right now your chance of survival is zero.”
• Level 6, “the Infinite Hell” (pp. 10–13): a secret sixth floor for life-sentenced/death-row prisoners whose deeds were “so heinous that the government has erased their very existences.” Bon Clay: “There’s a Level 6?!”

• Ace’s execution clock (pp. 13–17): Ace will be removed from Impel Down “in seven or eight hours” (the morning), transported to Navy Headquarters, and publicly executed “tomorrow at three in the afternoon.” He and Luffy are reaffirmed to be brothers; Jimbei is held in the adjacent Level 6 cell.
• Luffy is cured (pp. 18–19): after ~20 hours of screaming, Luffy bursts his chains and revives shouting “FOOD!!!” in under a full day — beating Ivankov’s own grim odds.
