Why this matters

Re-frames Ch. 134’s kingdom of doctors / one doctor left premise. Up to Ch. 136 the reader has understood Drum’s medical destitution as a consequence of Wapol’s flight (he abandoned the kingdom; the doctors followed him out / were destroyed by the Blackbeard conquest). Ch. 136 reveals the truer cause: Drum lost its doctors because Wapol expelled them by design. The medical-monopoly was the regime’s instrument of social control — the kingdom’s Great Medical Land reputation (Ch. 136 p. 9 caption) was a cover for a hostage regime where the country’s twenty doctors were kept in the castle research laboratory and every other doctor was expelled. The sick had to beg the king for treatment.

This is one of the most political-economy load-bearing reveals in the East-Blue-to-Alabasta arc. It installs the pattern of the king-as-medical-monopoly-extractor (the king controls health, so the king controls who lives) that the Drum arc will resolve by deposing the king and re-distributing medical knowledge to the people via Hiriluk’s quack-doctor-of-the-common-people lineage and the recruitment of Chopper as the first Strawhat doctor who does not gatekeep.
