This is a composition-inversion mirror, not a thematic rhyme. Hold the two panels side by side and the structural unit is identical — a tall, dark double-breasted-coated Impel Down power-figure with a distinctive oversized silhouette head, drawn vertically to dominate the panel, with a downed Luffy at his feet and haze at the base — while every load-bearing value flips:

Oda reuses the exact staging that introduced the man who poisoned Luffy to introduce the (still-faceless) figure who will cure him: the antidote is framed in the venom’s blocking. The silhouette tease lands harder precisely because the reader has just seen this composition mean “doom” — so the identical frame, re-pointed toward rescue, is the page’s whole emotional turn.
Watch-for (the lesson): when a tall, ornately-coated power-figure is staged towering over a downed Luffy with a big-silhouette head and haze at the base, log the composition — Oda likes to answer that “threat over the fallen hero” frame with its inverse (same blocking, opposite role) a few chapters later, and the recurring frame is itself the tell that the looming silhouette is someone the story has already named as Luffy’s only way out.
